Main reason for visit:

Often patients decide to have an eye examination because they have noticed a specific problem. This could be as simple as scratched spectacles or more seriously, a sudden loss of vision. Other patients just like regular examinations. Whatever the reason, we will always be happy to see you.

Eye Health:

Any history of eye health problems past or present is important. If you want to find out more about eye conditions

Visual problems:

Have you found any difficulty focussing
e.g. Teletext or small print?

Do you currently wear Spectacles or Contact Lenses?

Are you happy with your distance and near vision?
Is there anything you feel you should be seeing better with your spectacles or contact lenses?
